A QR Code is a code that can be scanned into Tomodachi Life, Miitomo, and Miitopia as an alternative to creating a Mii or transferring them from Mii Maker. This method allows for Miis to be imported from other islands while retaining the personality of said Mii.
To scan them, go to town hall in your Tomodachi Life game and choose the "QR Code" option. Choose "Scan QR Code" and then hold your 3DS or 2DS up to your computer monitor (or tablet or phone) so that the QR code shown lines up with the dotted outline shown on your 3DS.

Miitomo was a life-simulation game released for mobile devices, and a spin-off to the Tomodachi series. The player could create their own Mii and socialize with other players, or import up to 104 other Miis as "Sidekick Miis".
This usually happens randomly, just wait until the mii has that orange symbol with a smiley face on the window and hope said mii wants to be friends with the one you want. They can also be friends if someone introduces them to each other.
You can't assign family relationships to your non main mii in Tomodachi Collection either. It becomes weird when miis you don't want getting involved because they are related in real life or are characters you don't want together start confessing their love.
